Policies & Waivers
WAIVER OF LIABILITY
I agree that I shall provide health insurance to cover any personal injury and property damage sustained by the skater while participating in any activities of or while on the premises of leased or otherwise under the control of Chris Dyment Hockey. The undersigned assumes all responsibility for any and all risk of damage or injury that may occur to the skater named below as a participant in Chris Dyment Hockey clinics, practices, camps, scrimmages, games, skills sessions and other activities related to the program. Additionally, the undersigned hereby releases and discharges the program, Chris Dyment, its operators, employees, agents, supervisors, instructors and other players from all claims, demands, rights or causes of action present or future, whether known or anticipated and resulting from or arising out of an incident to the undersigned participation in said program. This is also my permission to have my child admitted and attended to for medical and dental treatment, in case of sickness or injury, that all physicals and inoculations are up to date. Chris Dyment Hockey has a zero tolerance policy with respect to uncontrollable behavior, bullying, hazing. Any skater found to have committed these acts will be immediately dismissed from the program and will forfeit all amounts paid. By signing this release and by being enrolled in this program you assent to the enforcement of this. Dates and times are subject to change due to weather and rink malfunctions. I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state in which the camp is taking place and agree that if any portion of this agreement is invalid, the remainder shall continue in full legal force and effect. I further agree that any legal proceedings related to this waiver shall take place in Boston, Commonwealth of Massachusetts. We understand that by being in a public space, we are exposed to communicable diseases such as COVID-19 and will not hold CDH or any of its employees responsible or liable for potential exposure to or contraction of such illnesses. This waiver is valid for any current and future lessons and camps with CDH.
WAIVER/PHOTOGRAPH RELEASE
I authorize Chris Dyment Hockey to use photos, and/or other likeness of myself and/or my child or the child for whom I have legal guardianship for any promotional materials regarding Chris Dyment Hockey programs. Such likeness will not be sold to other parties. Promotional materials bearing these likenesses may be distributed for free to the public and posted on the Chris Dyment Hockey's website, as well as social media pages for Chris Dyment Hockey. Chris Dyment Hockey reserves the right to use any photo or likeness for a time period beginning when this form is signed and ending upon written request of participant, parent or legal guardian.
MAKEUPS / PRO-RATE
No makeups, pro-rating allowed for February Vacation Camps. CDH allows one makeup for any missed session during the summer only due to vacations, illness. Please email erica@chrisdymenthockey.com for makeups. We do not prorate for missed sessions. We strive to keep our instructor to skater ratio low, so there are only so many "slots" allotted for each camp.
CANCELLATION POLICY
Full payment is required at time of registration. Cancellation requests received via email at least 30 days prior to camp start date, will be refunded, less a $25 administration fee. Cancellations made within 30 days of the camp start date will not be refunded. No shows will also not be refunded. If a player becomes sick or injured at or before camp, the unused portion of the clinic tuition will be returned in a letter of credit (voucher) for a future camp. A Doctors note is required. Chris Dyment Hockey can not provide refunds for cancellations due to unforeseen acts of God, including facility damage/closing complications or weather/COVID-19 related rink/facility cancellations and closures. If Chris Dyment Hockey cancels for any other reason not specified above we will offer you a refund. Chris Dyment Hockey reserves the right to cancel or change clinics, dates, times or locations.
THIS POLICY WILL BE STRICTLY ENFORCED. WE WILL NOT ISSUE REFUNDS OR CREDITS FOR SCHEDULING CONFLICTS.
